Since you're using the bag, just try switching to leaf. The fact that the grassiness only shows up over time is what makes me think you are not completely removing all of the hop material. The leafs should have fewer small pieces to escape your bag. An extra racking step could help, as could cold-crashing before racking.

Also, I'm assuming you're using reasonably fresh hops, and not something that's been in your freezer for 3 years.
