JP's Bourbon Oak aged Oat Stout Recipe

I can't find my copy of Zymurgy with JP's oaked oatmeal stout recipe. I know I had it at one point but, thanks to my incredibly haphazard filing system, it is misplaced. Does anyone have JP's recipe written down?

9.5 lbs British Pale
1 lb Flaked Oats
12 oz Crystal 75
12 oz Pale Chocolate
6 oz CaraFoam
2 oz Black Roasted Barley
4 oz Carafa II
4 oz White Wheat
4 oz Cacao Nibs
2 oz French Oak Cubes

1.5 oz Northern Brewer @ 60
1.5 oz British Kent Goldings @ 5

Estimated original gravity (OG): 1.57-61
Estimated SRM: 36
Estimated IBU’s: 43-47
Estimated alcohol ABV %: 5.6
Suggested fermentation temperature: 64

I add one ounce of oak to the fermenter. Another ounce was added to the keg. The beer was always in contact with the oak.

To hit those #s I have it at 5 gallons at 65%. Even if I set my program to 100% efficiency, I'm 10 points off at 10 gallons
