Sounds to me like the line is just a little short (maybe a foot). An easy way to test that out would be to turn the regulator down to 8 or 10 psi, vent the keg, and pour yourself a glass. If the beer comes out without excessive head, you know that 12 psi is just a bit too high for the 5 ft length of tubing.

I am no expert, but usually I dispense at 5-6 psi. The only time I changed that was when I had verticle lines running up from my basement. Even then I think I only ran about 10psi. It was about 10 feet of hose and seemed to pour slow but nicely.
